You will need to enter your Name, MRN (Optional), Employee ID #, Daytime phone #, email address where you can be reached, and any comments if you wish.
Please do not be late for your appointment as you may need to be rescheduled depending on your arrival time.
ALSO PLEASE DO NOT BRING CHILD / CHILDREN WITH YOU AS YOUR APPOINTMENT WILL NEED TO BE RESCHEDULED.
